For New Evaluators & Treatment Providers

1.  You must complete a fingerprint process and background check. The fee is $49.50. 

2. APPLICATION 1 - First application for Associate Level - For Associate Level Treatment Provider and/or Evaluator Request a Provider Database Management System (PDMS) Account.

  • You will receive an email with a link to activate your account and begin your application.

3. Complete the following competency-based forms and assessments based on the type of listing you are applying for. Read the Competency Assessment Summary for additional guidance.

For Existing Evaluators & Treatment Providers

Fill out the appropriate application:

  • APPLICATION 2 - Initial 3-year Associate and/or Change of Status application - This application is used for the following:
    • Providers who have completed their first year listing and are ready to apply for their initial 3 year listing
    • Providers ready to move to Full Operating Level prior to their renewal date (including DD/IS Specialty) or applying for Clinical Supervisor Status.
    • Submit the $125 application fee 
       
  • APPLICATION 3 - Renewal of current listing - This application is used for the following: 
    • For Associate and Full Operating Level Treatment Provider, Evaluator, and/or Clinical Supervisor applying to continue their status after their initial 3-year listing

To Add an Additional Status

1. Fill out APPLICATION 1 - First application for Associate Level - For Associate Level Treatment Provider and/or Evaluator Request a Provider Database Management System (PDMS) Account.

For Polygraph Examiners

Please use the following forms to apply, re-apply, move up to Full Operating Level, and/or comply with quality assurance requirements:

Application Fees

Existing Providers:

  • $125 fee for background investigation

Providers Not Currently Practicing:

  • $25 administration fee

*Provider fingerprint/background fees are paid to the vendor.

*Please attach verification of payment to your application.

The Approved Provider List identifies the providers who are eligible to receive referrals to provide evaluation, treatment, and assessment services to sex offenders in Colorado.
