2. APPLICATION 1 - First application for Associate Level - For Associate Level Treatment Provider and/or Evaluator Request a Provider Database Management System (PDMS) Account.
- You will receive an email with a link to activate your account and begin your application.
3. Complete the following competency-based forms and assessments based on the type of listing you are applying for. Read the Competency Assessment Summary for additional guidance.

Fill out the appropriate application:
- APPLICATION 2 - Initial 3-year Associate and/or Change of Status application - This application is used for the following:
- Providers who have completed their first year listing and are ready to apply for their initial 3 year listing
- Providers ready to move to Full Operating Level prior to their renewal date (including DD/IS Specialty) or applying for Clinical Supervisor Status.
- Submit the $125 application fee
- APPLICATION 3 - Renewal of current listing - This application is used for the following:
- For Associate and Full Operating Level Treatment Provider, Evaluator, and/or Clinical Supervisor applying to continue their status after their initial 3-year listing

The Approved Provider List
The Approved Provider List identifies the providers who are eligible to receive referrals to provide evaluation, treatment, and assessment services to sex offenders in Colorado.
